CCPA Privacy Request Form

Your privacy request processed as part of the California Consumer Privacy Act.

Use this form to submit a request relating to your personal information. Our team may contact you to request additional information to verify your identity or authorization to submit this request.  If we cannot verify your identity, we will not be able to process your request. Any new information collected will only be used for verification and security or fraud-prevention purposes. We will delete any new personal information we collect about you, unless we are required by law to keep it. Please note that certain requests may require that you take additional steps to confirm your request.

Please note: If you are trying to submit a request on behalf of your child, and you have NOT registered as the parent, then you must contact the child's art teacher and register as the parent PRIOR to submitting a request. This method will ensure and authorize you as the parent of the child. Alternatively, you may contact Artsonia, and we can provide a form to fill out, which will require a certified birth certificate and government issued ID of the parent.
